SS-31 / elamipretide: biology and human evidence
A small peptide studied for its interaction with parts of the cell’s energy system. A four-residue synthetic peptide commonly called elamipretide. Research-code similarity does not prove a seller’s vial is the licensed medicine.
The biology, in plain words.
SS-31 is commonly called elamipretide. It is a synthetic four-residue peptide that interacts with cardiolipin, a fat-like molecule in the inner membrane of mitochondria. That membrane is important to the machinery that converts fuel into usable cell energy. The research is therefore about a specific molecular setting, not a general energy boost. A code name, a raw molecule and a licensed medicine are separate levels of identity. The exact chemical form and finished formulation still matter.
Walking distance in six minutes and a patient-reported fatigue scale were the main outcomes in MMPOWER-3. The trial enrolled 218 people with primary mitochondrial myopathy. It did not meet either main endpoint, which limits broad claims based on the same compound’s mitochondrial mechanism.
A primary endpoint is the main outcome a trial is designed to test. Secondary outcomes and subgroup findings can help guide later studies, but they do not make an unsuccessful primary endpoint disappear. This distinction matters when discussing mitochondrial trial results.
What the studies found.
In 218 participants with primary mitochondrial myopathy, the 2023 MMPOWER-3 trial missed its main walking-distance and fatigue endpoints. Separately, the US approved Forzinity in 2025 for a narrow Barth-syndrome indication through accelerated approval. Neither result establishes a broad wellness benefit or equivalence of a proposed SS-31 blend.
Details that change the comparison.
SS-31 is a research code commonly associated with elamipretide. A vial using that code is not automatically equivalent to Forzinity, a particular finished medicine. Molecular stereochemistry, terminal groups, salt, actual content and formulation are part of product identity. A name match alone does not establish equivalence.
Barth syndrome and primary mitochondrial myopathy are different disease contexts. The US accelerated approval is narrow and includes a requirement for confirmatory evidence. It should not be converted into a general longevity or energy claim. Similarly, the negative primary endpoints in MMPOWER-3 remain part of the record; selecting only favorable secondary findings would give an incomplete view.
